Position: CARE MANAGER - PROJECT: ELEVATE
This position does not offer remote days and the in-office schedule is Tuesday - Saturday (8-hour shifts)

Job Description
Care Manager utilizes participant-centered, trauma informed, harm reduction and strengths-based approaches to empower individuals to seek liberation and reduce recidivism. Care Manager works with individuals in a Community Corrections facility setting. Care manager collaborates with partners and other community-based service providers to ensure participants have low-barrier access to services.  Care Manager is knowledgeable in the many avenues of healing from trauma and substance use disorders while implementing culturally responsive, gender-responsive, and trauma-informed care.

Project: Elevate Care Manager is responsible for:
  • Providing intensive, holistic care management and transition planning services for residents serving a sentence in a Community Correction setting.
  • Understanding and implementing the Project: Elevate Scope of Work and completing all required assessments, documentation, care plans and reports in accordance with Community Corrections Standards.
  • Providing “in-reach” – including care management activities - to individuals incarcerated in jail or prison prior to their arrival at Project: Elevate.
  • Communicating and collaborating with operations staff, partners, and other community-based service providers to ensure participants have low-barrier access to services.
  • Maintaining accurate and timely data in compliance with Community Corrections Standards.
  • Identifying and connecting participants with appropriate referrals to community mental health and supportive services providers and permanent housing resources.
  • Conduct behavioral interventions in accordance with evidence-based practices

Other Responsibilities Include:
  • Attending all required meetings
  • Obtaining all necessary and required supervision and training
  • Maintaining Self-Care
  • Other duties as assigned

Qualifications:
  • Must have a valid driver’s license or state Identification
  • Lived experience navigating the Criminal/Legal Justice System is preferred.
  • Restorative Justice knowledge and skills are preferred
  • Working understanding of harm reduction theory, trauma informed services, and restorative justice
  • At least 2 years of experience working with a diverse population with behavioral health needs is required, 5 years preferred.
  • Excellent communication, interpersonal and team building skills

Salary is $60,000 annually
